Amoebic dysentery Diarrhea, blood and mucus in stool with no fever and positive stool microscopy
Bacillary dysentery Diarrhoea, abdominal pain, mucus or blood in stools, dehydration and enterobacterial infections
Gastroenteritis Diarrhoea (small bowel or large bowel) and dehydration
Hepatitis Fatigue, abdominal pain, jaundice and deranged liver function tests
Cholecystitis Right upper quadrant abdominal pain and tenderness, fever and/or bilious vomit
Worm infestations Generalised abdominal pain, nausea, history of worm in stools and laboratory confirmation from stool samples
Coeliac disease Chronic diarrhoea, abdominal distension, fatigue and weight loss
Crohn’s disease Chronic abdominal pain, diarrhoea, weight loss and evidence of chronic inflammatory condition
Irritable bowel syndrome Chronic abdominal pain, bloating, flatulence, altered bowel habits but not evidence of infective or inflammatory pathology
Gastritis Epigastric burning sensation or pain and early satiety
Ascites Distended abdomen with no rigidity, evidence of fluid collection in peritoneum and underlying pathology.

Infectious diseases

Dengue High grade fever with chills, patient from endemic area and positive peripheral smear test
Filariasis Dry cough associated with groin pain, leg swellings and patient from endemic areas
Leptospirosis High grade fever, muscle ache, jaundice, red eyes, history of close association with animals and birds or prolonged exposure to contaminated water
Lyme disease Fever, circular skin rash (erythema migrans), arthralgia, fatigue, joint pains, history of tick bites.
Malaria High grade fever with chills, patient from endemic area and positive peripheral smear test
Measles Rhinitis, red eyes, small greyish-white spots on the buccal mucosa (Koplik spots) and blotchy maculopapular rash
Typhoid High grade fever, PR bleeding, loss of appetite, weakness, red spots on abdomen and positive Widal test
Whooping cough Prolonged cough (classic whoop sound), low grade fever and apnoeic spells in infants

Other conditions

Renal stones Intermittent abdominal or flank pain, increased urinary frequency, burning urination and associated radiological evidence
Urinary tract infections Fever, chills, flank pain, burning urination and increased urinary frequency
Scorpion sting History of bites, localized swelling, bite marks and evidence to toxicity
Venomous bites History of bites, localized swelling, bite marks and evidence to toxicity
Migraine Severe headaches, increased sensitivity to light or sound but no evidence of systemic infections
Diabetic ketoacidosis Increased thirst, urinary frequency and abdominal pain with high levels of blood sugars, ketones in urine
Gestational diabetes Pregnant patients with excessive weight gain, fatigue, increased urinary frequency with abnormal glucose tolerance test
