Laryngotracheitis (Croup) is the viral infection of the upper respiratory tract causing inflammation of the larynx and trachea and varying degrees of airway obstruction. It is the most common cause of stridor in children.
Medications:
- Hospitalize the patient
- Give humidified oxygen
- Inj. Dexa (Dexamethasone 2 mg/2ml) – 0.15 – 0.60 mg/kg to be given i.v. (maximum dose 16 mg).
- Respule Pulmicort (Budesonide 1 mg/2ml) – give 4 ml (2 mg) via nebuliser twice a day.
- Inj. Vasocon (Adrenaline 1 mg/ml 1:1000) 0.5 ml in 5 ml saline via nebuliser. Continue or repeat this as needed till systemic effects of adrenaline are observed.
- Tab. Moxikind-CV-625 (Amoxicillin 500 mg, Clavulanic acid 125 mg) 1 tablet thrice a day for 10 days for suspected bacterial infection.
- Cap. Velgut (probiotic and prebiotic) 1 capsule twice a day for 10 days.
Investigations:
- Complete blood count – for systemic infections
- X ray chest – look for pencil sign of proximal trachea on the AP view
Referral:
Consider further referral to ENT specialist or secondary care in case of clinical deterioration or if patient has any other complications.
