Skin conditions

Lice infestation Itching in scalp, pubis and evidence of lice organisms, irritability and lack of sleep
Prickly heat Itchy red rash on the skin on exposure to hot temperature and no signs of infections
Psoriasis Localised redness of skin, dry and flaky skin with silvery scales
Scabies Intense night-time itching, burrow tracks in web spaces of hands, feet, other skin folds with positive microscopy
Tinea (ring worm infestation) Localised symptoms with red scaly rash, blisters, thick and discoloured nails
Superficial skin infection Swelling, redness and skin warmth. Patients may present with systemic evidence of infection.
Eczema Patchy dry skin, discolouration, localised swelling but no evidence of skin infection.
Intertrigo Reddish skin rash, induced or aggravated by heat, moisture, maceration or friction in skin folds
Skin allergy (urticaria) Red discolouration of skin, skin elevation, blisters and history of contact to sensitive products
Bee sting or insect bites History of bites, localised pain, stinging, raised skin or evidence of secondary infection

Infective conditions

Chicken pox Fever, skin rash with or without blisters, headache and tiredness
Filariasis Dry cough associated with groin pain, leg swellings. This is more common in endemic areas.
Measles Rhinitis, red eyes, small greyish-white spots on the buccal mucosa (Koplik spots) and blotchy maculopapular rash
Worm infestations Generalised abdominal pain, nausea, history of worm in stools and laboratory confirmation from stool samples

Metabolic conditions

Hepatitis Fatigue, nausea, vomiting, abdominal pain, jaundice and deranged liver function tests
Addison’s disease Hypotension, hypoglycaemic episodes, fatigue, nausea and vomiting
Hypoparathyroidism Fatigue, muscle twitching, cramps and low levels of parathyroid hormone
Hypothyroidism Fatigue, weight gain, intolerance to cold, dry skin and abnormal thyroid function test
Crohn’s disease Bloating, flatulence, diarrhoea, evidence of malabsorption but no evidence of infective pathology

Others

Eosinophilia Persistent cough, breathlessness and laboratory confirmation of raised eosinophils
Varicose veins Engorged veins, localised aching, skin discolouration, muscle cramps and evidence of reduced venous drainage
Venous thrombosis Localised swelling, pain, skin discolouration and warm skin
Cholecystitis Localised pain around right upper quadrant with rigid abdomen, fever along with radiological evidence of inflamed gall bladder with or without gall stones
Ascites Distended abdomen with no rigidity, evidence of fluid collection in peritoneum and underlying pathology
Vaginitis Vaginal discharge, dryness, painful intercourse and spotting
Gout Joint swelling, pain in joints, reddish discolouration and localised tenderness along with uric acid crystals in joint aspirate
