Symptomatic treatments:

  • Ensure good blood pressure management
  • Tab. Gemvert (Cinnarizine 20 mg + Dimenhydrinate 40 mg) – Dr. Reddy lab – 83/- -one tablet thrice a day for 3 days.
  • Ensure adequate hydration – Electral ORS – 1 sachet in 1 litre of water twice a day.

 Common conditions to consider:

Condition

Common clinical presentation along with dizziness

Vertigo Abnormality in the auditory canal, vestibular nerve or brain stem
Labyrinthitis Reduced hearing, earache or severe nausea
Dehydration Dizziness could be a predominant symptoms for patients with dehydration. Commonest causes of dehydration could be amoebic dysentery, bacillary dysentery, gastroenteritis or due to infectious diseases.
Anaemia Fatigue, palor, breathlessness and/or evidence of blood loss
Abnormal uterine bleeding Menorrhagia or polymenorrhagia (dizziness and anaemia could be presenting complaints due to cultural barriers).
Hypotension Sudden changes in anti-hypertensives
Congestive cardiac failure Dizziness is postural with pedal oedema, irregular rhythm and 2 D echo evidence of reduced ejection fraction
Addison’s disease Weight loss, changes in skin colour, fainting and associated laboratory evidence
Hypoglycaemia Prolonged fasting, overactivity, sudden changes or non-compliance to anti-diabetic medications
Hypoparathyroidism Cramps, weakness, fatigue and reduced serum calcium and parathyroid hormone
Transient ischaemic attack Short term double vision, transient blindness, slurred speech
Stroke New onset neurological deficit, severe headache, slurred speech
Parkinson’s disease Resting tremor, difficulty in speech, generalised rigidity
Peripheral neuropathy Peripheral loss of sensations, imbalance, pins and needles sensation in extremities
Fibromyalgia Generalised pain, fatigue and muscle weakness

Note: Any generalised or significant illness can lead to dizziness and hence a detailed history, examination and investigation is needed.
