Gastroesophageal reflux is characterized by reflux of stomach contents back up into the esophagus. Patients often present with regurgitation of food, burping, chest discomfort, sore throat, lump in throat, bad breath or sour taste. It is important to rule out cardiac pathologies in vulnerable patients.

Medications:

  • Cap. Pantakind DSR (Pantoprazole 40 mg, Domperidone 30 mg) 1 capsule once a day for 4 weeks followed by
  • Tab. Pantakind 20 (Pantoprazole 20 mg) 1 tablet once a day for 4 weeks.
  • Syrup. Gelusil MPS (Aluminium hydroxide, Magnesium hydroxide, Simethicone) 15 mls thrice a day for 7 days.

For Helicobacter pylori infection

  • Tab. Pantocid HP Combipack  (Amoxicillin 750 mg, Pantoprazole 40 mg, Clarithromycin 500 mg) twice a day for 14 days.

General advice:

  • Maintain adequate hydration
  • Small bland meals
  • Advice weight reduction
  • Sleep in a propped up and lateral position.
  • Avoid sleeping for 2 hours after meal.
  • Avoid tobacco, coffee, tea, chocolate, spices, fatty food or dairy products.
  • Avoid tight fitting clothes
  • Avoid non-steroidal anti-inflammatory drugs

Investigations:

  • OGD (oesophago-gastro-duodeno-scopy) – to look for ulcers and inflammation due to acid reflux.
  • Stool biochemistry – to identify antigens for H.pylori infection.

Referral:

Consider referral to gastroenterologist if symptoms persist inspite of the above treatments or if patient presents with weight loss.
