Paronychia is an infection around a fingernail causing swelling, redness and pain and can be caused by bacterial or fungal in origin. It is common in patients with persistently wet digits such as housemaids, farmers etc. Sever cases can lead to localised abscess.
Medications:
- Cream Triben XT New (Clobetasol 0.05%, Ofloxacin 0.75%, Terbinafine 1%) to be applied to affected area twice a day.
- Tab. Odoxil 500 (Cefadroxil 500 mg) – 1 tablet twice a day for 10 days.
- Tab. Forcan 150 (Fluconazole 150 mg) – 1 tablet once only.
Supplementary treatment:
- Tab. Flanzen 10 (Serratiopeptidase 10 mg) – 1 tablet thrice a day to reduce swelling.
- Tab. Calpol 650 (Paracetamol 650 mg) 1 tablet 4 times a day.
- Incision and drainage of abscess if needed.
General advice:
- Keep affected area clean and dry.
- Trim the nails in semilunar shape.
- Avoid biting nails.
- Use gloves and avoid keeping the hands wet.
- Apply moist heat to the area.
Investigations:
- Complete blood count – to rule out systemic infection.
- Abscess culuture and sensitivity – to optimise antibiotic treatment.
Referral:
Consider referring to surgeon for incision and drainage of abscess. Severe cases may need removal of infected nail.
