Pelvic inflammatory disease is characterized by infection of the female reproductive organs i.e uterus, fallopian tubes, ovaries and cervix. It is usually caused by sexually transmitted diseases like chlamydia, gonorrhea and need antibiotics. Symptoms include abdominal pain, bleeding between the periods, heavy vaginal discharge, pain during sex, fever with chills and pain while passing urine. Long standing and untreated infection can cause infertility.

Medications:

  • Tab. Taxim-O 200 (Cefixime 200 mg) 1 tablet twice a day for 5 days.
  • Cap. Microdox-LBS (Doxycycline 100 mg, Lactobacillus) 1 capsule twice a day for 14 days.
  • Tab. Flagyl 400 (Metronidazole 400 mg) 1 tablet twice a day for 14 days.

Supportive therapies:

  • Tab. Cyclopam (Dicyclomine 20 mg, Paracetamol 500 mg) one tablet 4 times a day for colicky pain.
  • Liquid wash V-Wash Plus (Lactic acid-based soap solution) for perineal and vaginal cleaning.
  • Sexual partner needs to be treated with antibiotics.

General advice:

  • Advice for safe sex, using barrier contraceptives.
  • Maintain good personal hygiene and cleanliness of perineal area.

Investigations:

  • Complete blood count – to rule out infection
  • CRP – to assess infective process
  • Vaginal swab test – to identify trichomonas or bacterial vaginosis and antibiotic sensitivity.
  • Mid-stream urine for urine routine and microscopy – to rule out associated urinary infection.
  • Ultrasound pelvis and abdomen – to rule out complications like adhesions or other causes for abdominal pain.

Referral:

Consider referral to gynecologist if patient fails to respond to the treatments or presents with additional complications.
